Forms
Most of our forms are available in either Microsoft Word ( .doc file) or Adobe Acrobat ( .pdf file) formats.
Being smaller, the Acrobat files will download faster, but your computer requires Adobe Acrobat Reader to open them. The best version for your operating system can be downloaded free of charge from the Adobe web site.

Completed forms should be returned to the Academy by post
or fax.
Desktop Wallpaper

You can set the Academy's logo as your desktop background. Download here (39 kB).
The file will open in your browser, then save it to an easily-accessible folder. To set up the wallpaper in Windows, right-click the downloaded file and select the option to save as wallpaper.
Alternatively, you can set it up using the Display settings in the Control Panel (Start > Settings > Control Panel > Display). Select the Desktop tab, click the Browse button to find the file, select it, click Apply and then OK.

Gippstar Nomination Forms
The Gippstar Awards are designed to recognise sporting excellence of Gippsland's sportsmen and sports women, amateur or professional, of any age.
Award winners are chosen from public recommendations, so nominations of outstanding sporting achievement are invited.
